- Signed PDF (with optional printed copy)

📄 Disability Benefits Questionnaires (DBQs)
DBQs are standardized VA forms used to document the severity and impact of a diagnosed condition. Our team completes these forms accurately and clearly, ensuring all relevant symptoms and limitations are properly reported.
Includes:
- Evaluation of each claimed condition
- Completed and signed DBQ form
- Telehealth (for CA residents) or in-person completion

🔍 Medical Record Review

🩺 In-Person Medical Exam
Certain claims require a physical examination to support a DBQ or Nexus Letter. We offer in-person exams at our clinic in Corona, California.
Includes:
- Comprehensive physical evaluation
- Functional testing (as appropriate)
- Use with DBQs and/or Nexus documentation
Note: Telehealth is only available to California residents. In-person exams are required for some musculoskeletal, neurological, and skin conditions.
